A bug report template that makes issues reproducible: expected vs actual, environment, steps, and impact. Copy/paste-ready.

Most “bugs” become expensive because they’re not reproducible. This template makes reproduction the default.

Use it for client-reported issues or internal QA.

  • Title: (one line)
  • Environment: Production / Staging / Other
  • URL(s):
  • Severity: Low / Medium / High

Expected vs actual

  • Expected: what should happen
  • Actual: what happened instead

Steps to reproduce

Impact

  • Who is affected?
  • How often does it happen?
  • Is there a workaround?

Evidence

  • Screenshot/video:
  • Error message text (copy/paste):
  • Console/network logs (if relevant):

Access (only if needed)

  • Do we have credentials to reproduce? Yes / No
  • If yes, where should we receive them securely?
